At your appointment, we will have your completed form available for you to review. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

PLEASE NOTE:  Our form has room for only three medications, so if you take more than three, please bring your full list of medications on paper so we can scan it into your chart.
